Clinical Assessment and Treatment Services
Within the virtual care business unit we provide Clinical Assessment & Treatment Services (CATS). This includes remote clinical triage of GP Out of Hours referrals, as well as our Clinical Assessment Services (CAS)
Our highly skilled team provide additional clinical validation for patients who are referred via NHS111, 999 or by another healthcare professional. We make sure that patients receive the most appropriate onward care and can refer into a wide range of services
45% of the CAS patients that we see each month would otherwise have called 999 or attended the local Emergency Department
Our CAS uses intelligent automation for waiting list management. Based on real time data this optimises patient flow and makes the most of clinical capacity so that the most urgent patients are seen first
We have provided call handling services since 2011
Currently our 115 call handlers support North West Ambulance Service (NWAS) by answering over 500,000 NHS 111 calls each year
We have a dedicated call centre full of hard working, compassionate people that provide high quality care to callers. These call handlers work closely with clinical teams to maintain oversight and make sure that patients are receiving the right level of care in the most appropriate setting
In addition to 111 call handling services we also provide a local booking service “0300” which supports with onward referral of patients and direct booking into local services

Remote Monitoring & Virtual Wards
FCMS is an experienced provider of virtual ward and remote monitoring services. We have worked with a number of secondary care providers to deliver virtual ward services since the pandemic. These services help to prevent avoidable admissions, or to support timely discharge by providing specialist care and clinical oversight for patients in their own home
Through our remote monitoring services we provide telehealth services where patients with long-term conditions record their own observations and enter them into an app with the readings then closely monitored and reviewed by a clinician
This service is for high risk patients who have Covid-19
All patients are closely monitored for deterioration and onward referral/escalation and where appropriate prescribed anti-viral medication
As with our other virtual care services this is aimed at providing care for patients in the comfort of their own home whilst avoiding unnecessary hospital attendances

Care Coordination
Care Coordination is a single point of access, admission avoidance service which supports patients with complex healthcare needs be cared for in their preferred surroundings, preventing unplanned hospital attendances
Care Coordination work closely with Trinity Hospice to support end-of-life care
The service is continually expanding as we identify new and innovative ways of meeting the needs of patients across the Fylde Coast
