Remote Home Monitoring & Virtual Wards
In 2020, FCMS began to deliver new Virtual Ward services for Covid patients and has since developed to include Acute Respiratory Infection and Cardiac (as part of an early discharge and waiting list management). We currently provide virtual ward services on both the Fylde Coast and Morecambe Bay.
Our dedicated team of clinicians support primary and hospital care with the ongoing management of these patients, ensuring they receive the care and supervision they need, all without requiring admission to hospital.
In August 2021, our remote monitoring service went live on the Fylde Coast. This service supports patients with long-term conditions, such as diabetes, heart failure, or COPD.
Utilising a telehealth platform, we offer a technology-enabled care solution, where we can clinically assess and monitor patients in the comfort of their own home. This in turn helps to manage hospital care capacity, by reducing both re-admissions and the time patients spend in hospital.
