At FCMS, we work really hard, love what we do and are seriously focused on our patients and colleagues. We thought the best way to get this message across would be to show you.
We are starting a spotlight series, where we will be focusing on different areas within the organisation, what they do, how they work, the team behind the service and how you can access them.
In this first edition, we will be focusing on…
Remote Home Monitoring & Virtual Wards
In 2020, FCMS began to deliver new Virtual Ward services for Covid patients and has since developed to include Acute Respiratory Infection and Cardiac (as part of an early discharge and waiting list management). We currently provide virtual ward services on both the Fylde Coast and Morecambe Bay.
Our dedicated team of clinicians support primary and hospital care with the ongoing management of these patients, ensuring they receive the care and supervision they need, all without requiring admission to hospital.
In August 2021, our remote monitoring service went live on the Fylde Coast. This service supports patients with long-term conditions, such as diabetes, heart failure, or COPD.
Utilising a telehealth platform, we offer a technology-enabled care solution, where we can clinically assess and monitor patients in the comfort of their own home. This in turn helps to manage hospital care capacity, by reducing both re-admissions and the time patients spend in hospital.
How it works
Below is a video that explains just how it all works and the benefits patients have that access the service.
We hope this has given you a helpful insight into what this service provides and how it operates.
FCMS provide everything from urgent care, diagnostic services, emergency dental services, call handling, targeted care for vulnerable communities and much much more.
We will continue to share with you, what we do, in the next edition of our spotlight series.
